ITEMS 1 to 6 ARE TO BE SUBMITTED IN TRIPLICATE (3 SETS)

1. Burial Transit Permit.

2. Certification of Embalming & Preparation of Human Remains.

3. Notarized statement of the Funeral Director (stating that the casket contains only the remains of the deceased and nothing else).

4. Medical Examiner’s Report (if applicable) if the deceased was killed.

5. Notarized copy of Death Certificate.

6. Certification of Vital Record regarding communicable/non-communicable disease(s) – [this is to state whether or not the deceased had contagious disease. If so, the type of disease should be stated in this document].

7. $30 US dollar, payable to the Embassy of Nigeria, Addis Abba, Ethiopia) as Attestation fee.

8. Nigerian Passport of the Deceased (in case passport is not available, the closest relative should send a notarized letter stating the same).

9. A waiver letter for the repatriation of human remains from the Federal Ministry of Health in Abuja, Nigeria.

NOTE: Allow 7 working days for processing time. Also, drop off and collection of documents from Monday through Thursday, between the hours of 11: 00 am to 12: 30 pm.

1. Formal request letter for the Emergency Travel Certificate by the applicant. Include your current

address and telephone number, please.

2. Expired Nigerian passport (to be sighted and returned to owner), as well as a copy of passport data page.

3. For a loss passport, Please include a police report as well as a data page of a lost Nigeria passport (if applicable).

4. 2 passport size photographs.

5. Copy of itinerary or airline ticket.

6. $150 US dollar, payable to the Embassy of Nigeria, Addis Abba, Ethiopia.

7. Applicants under the age of 18 (Eighteen):

a) Copy of Birth Certificate.

b) Copy of both parents’ Nigeria passport (data page). At least one parent must be Nigerian.

c) Letter of consent signed by both parents.

 

NOTE: The Travel Document is valid for 30 days only and cannot be used to return to Ethiopia (one-way journey). Allow 7 working days for processing time. Also, drop off and collection of documents from Monday through Thursday between hours of 11:00 am to 12:30 pm.

1. Application (on your company’s letterhead) stating your request from the Embassy and list of the products to be exported.

2. Certificates of free sale from the Departments of State (federal and state) where your company is located, respectively.

3. Two sets of the documents to be legalized, i.e. original and a copy for our record.

4. $30 per product/document only, payable to the Embassy of Nigeria Addis Abba, Ethiopia.
