REQUIREMENTS FOR CHANGE OF NAME

A written application for change of name is to be submitted with the following documents:

    1. Embassy ID.
    2. Birth Certificate.
    3. Marriage or Divorce Certificate (whichever is applicable).
    4. Affidavit of Change of Name.
    5. Newspaper publication of change of name in a National Newspaper in Nigeria.
    6. Photocopy of the data page of International Passport.
    7. Recent Utility Bill as proof of address (not exceeding the previous 3 months).
    8. Two (2) passport-sized photographs.
